Working for shorter sessions to provide community access for service users and respite for families.

Part-time including evenings and weekends

An opportunity has arisen to work as Assistant Outreach Coordinator within our diverse outreach department.
This post will involve working alongside the Outreach Manager and Outreach Co-ordinator; working in a person centered way. You will be a point of contact for staff, the people we support and their families. This role primarily involves being responsible for the coordination of shift cover, keeping these records in date and planning for future events, to ensure the service provided is efficient. As the assistant you will receive training to operate the Access Planning Software and Access Care Planning mobile app.
We are looking for someone with a ‘can do’ attitude and at short notice be prepared to work with the people we support in accordance with current best practice, according to policy and procedures, agreed standards, legislative requirements and relevant regulations under the direction of management.
You must have previous experience of working with people with a learning disability.
Applicant should be forward thinking and use all available technology ensuring service user data is current and updated regularly.
